In the Assassination perk tree, finally got down to it and a question mark is still next to it.

Nevermind, figured it out. Leaving this for other people who want to know.

Not sure if its 100% of the time, but backstabs seem to instantly kill. Did 2 in a row on the trolls with the big hammers, both of them died instantly.
