are puzzles and puzzle locations random or the same for all?

was wondering if the puzzles we get are in same locations and if they are random?

for example if I have a puzzle at 2nd floor, does everyone else also have the same puzzle at the same location? or is it randomly generated for every game?

WHich is very good. The developers can design better dungeon layouts, and puzzles, and have a lot more control over when you recieve items...which gives you a better experience.

Are the items really the same? That's a partial shame as you can takes skills knowing what you will come across. Would be better to say X will be a weapon, y is armor and so on to have some randomness.
