Map drawn on a note/scroll (lvl 4?)

I think I picked it up on level 4, anyway. But yeah, there's a hand-drawn portion of a map with a square room, and a symbol above one of the corners. I think the symbol is the same one that's above most of the doors. I looked at the map and didn't see a room that fit the drawing exactly (as far as layout) on either level 4 or 5. I suppose it's possible that I missed it, but I'm not sure. It's entirely possible that it's a secret I already found, I suppose. But I have no idea.
I'm about to start level 6 and I don't want to until I figure this out, since...
...since it appears level 6 locks you out of level 5, preventing backtracking.

if i remember correctly it actually maps...
also you shouldnt worry about that since
level 6 releases you once you are halfway through

If it is the map with the picture of the slime, then the entrance is on Level 3.
More specific location:
In the Northwest part of the map, hidden switch opens a door that leads back down to Level 4.

yeah it maps a location to the north-west of the map where you meet the ice 'rapters. one the wall there's a button with that symbol on it. press it and it opens a hidden wall near the start of that area (past the corridor with the two spinners that spin you 180 degrees) and through the openable/closable gate where it says you will be hunted down, look down the short twisty corridor.

If it's the map with the slime on it, it's for level 3.

It's the secret button on lvl 7 (western part) to alternate entrance to stairs to level 8.
This is by the room with sewer tentacle monster and a nearby fire demon.

I tried looking for part of the map that fits the configuration, on level 3, but nothing does. At least, not that I can tell. Here's a screencap of the note (it does have a slime):
