I seem to be stuck in level 9. I got past the sanctum with the three altars, found the lightning blade, but there seems to be no way forward (The central area didnt open when I reached it) and no way back (the door to the sanctum locked behind me.)
Am I missing something, or have I hit a bug?

no bug.
there is a secret button in the outer sanctum S room that opens that door. In order for the central area to open you must have stepped on pressure plates on the N, S, and E sides of the center. On the N side there is a lever in a nearby hall to open the door, on the S side there is a secret button in a nearby hall, on the E side there are two levers that, when both down, open the door.

I have already opened the door, and gone into the alcove. However, there is no way to go beyond that, because I have not yet been to the other two pressure plates, and there is no way back.
Edit: I went to one of the other pressure plates, past the snakes corridor. I think I had a key unused from earlier in the dungeon, however, allowing me early access to the sanctum.

I may not have been clear enough - there IS a way back, you can open the front door of the sanctum again
by pushing a secret button nearby
You'll just need to go back around to the N side

can you be more detailed? There is no levers puzzle in the north part of level 9 that I can see or think of. If you mean the row of levers that Toorum talks about in a note on lv10, then
look carefully at the exact placement of the pillars - they are slightly offset, so adjust the levers to copy the pillars relative to the wall

You need secret 3-1-12, also I've just added the first three levels walkthrough, see first post
