This is what I understood, but it is still not working. Does one have to have all of the alcoves filled prior to opening the room?
I found and have filled 9 alcoves in the skeleton cells, plus 1 nearby, 2 by the healing crystal 3 in the small maze west of the library, 6 to get into the library, 1 in the altar room (plus the altar itself for good measure) and 2 in the round key room. I also put items in all the spots behind the iron door just to cover all the bases. No teleporter. Did I miss a secret alcove somewhere?
My minor griping aside, I really am enjoying your mod thus far. I'm just wrapping up the next two levels and like the direction things are going. Well done.

No.
The alcoves:
1 at the start.
3 in the eastern corridor. (When you've teleported in.)
6 to open the library.
1 in the room opened with the gold key.
12 in the area opened by the brass key.
2 in the room opened by the round key.
That's actually 25 alcoves but only 23 is needed. (The first one to test the mod had problems with this and I lowered the number to make it easier.) It's also important to leave something in the alcove, not only put it there and then take it back. But I think you've figured that part out already.
You're not missing anything that's essential to the game if you don't figure the alcoves out. It's 1 secret and some extra stuff.

When I played your revised version I noted that
the teleporter was already active when I arrived at the round key room but placing an item on both the alcoves in that room deactivated it. So maybe removing an item might cure Lurker's problem?

Oh, I didn't think that far.
That's a problem I need to keep in mind.

Stuck on the second floor.
got three gems so far. It's the room with the demon heads shooting fireballs,and altar to the god of war. I noticed that put the axe or the red gems makes noise of cells open, but the way back is still blocked.

The way in is not the way out.
It's a breakable cave-in behind one of the doors that open. It's a lesson worth remembering, hit stuff and see if it breaks.

I'm giving up on that particular piece for the moment. I think there might be lingering bug in there somewhere.
Next problem is
I'm not sure what to put on the Altar of Pjofa on level 2. I've tried treasures, gems, gear keys (before I used them) and even the Key Sword I found later on. I thought Took (or whatever his name was in the catacombs) might have had something valuable but he only had a red gem.
In the same area, I see a blue gem in the teleporter maze that I can' reach it.
That is the last gem I need to open up all the iron doors on level 3. Am I missing a secret switch somewhere that will allow me to get it?
Finally, on level 4 in Arrak's hidden chambers. How do I get those two metal doors open? If these things are all tied together, only give me what I need to know. Thanks for the help.

You need the "Thief's Cloak".
It's in a wooden box at the beginning of level 2 by the feet of the ogre.
I see a blue gem in the teleporter maze that I can' reach.
Yes, you've missed a lever.
At the northern side of the teleporter maze there are two alcoves. Removing what's in them and putting something back will open a sectret door where you'll find the lever.
Finally, on level 4 in Arrak's hidden chambers. How do I get those two metal doors open?
This one is sneaky, pay attention to details in places you don't normally look at.
One of the the golden statues is holding a treasure. Remove it and the metal doors will open.

... with 200 evasion. Holy crap! Actually a cloak was one of the first things I tried but I didn't realize the significance of that prize.

Yep, that was very hard to see. I went around the room a few times before I saw a glint of it.
Right now I'm going through the
rites of passage and have just finished going through the earth level.
Based upon that I'm guessing I missed the two blood skulls in the really, really, really frustrating teleporter area. Did I mention that it was frustrating?
Hints?
Meanwhile, some things I've passed over that I couldn't figure out:
Level 2
I hear an ogre in the northwest portion of the map where I have an unexplored area. Did I miss a hidden switch?
Level 3
The last Iron Door just before going down to level 4. No ideas on this one.
Level 5
Again with the Iron Door. I'm guessing with this one though that I just missed one of the numerous hidden switches present on this level.

I've had moments when I've hated myself for creating the teleporter area. I won't do anything like it ever again. I think.
It's the same principle as in the room with the golden statues, look in their hands. The skulls are so dark that they are a bit hard to see. I never came around to giving them some sort of "glitter" effect.
Level 2
Remember the text on the wall telling you to not feed the prisoners or you'll be punished too? An ogre can deal a lot of punishment...
Totally spoiling it:
Put something in through the grate and you'll be teleported to the area where the ogre is. Pull the right levers and you can get away without letting him out. Pull the wrong ones and it will hurt... a bit.
Level 3
In the area for the "good students" you open a steel door and find four alcoves. And the first "lesson" is to give...
Level 5
Yes. In the middle of the map there's and alcove...
