Tetris map in Sewers?

Can anyone give me an idea of where the Tetris map is?
I know what to do just have not been able to find the map.
I've been in all rooms in sewer I believe, only thing I haven't done there is Tetris and beat that laughing bastard AGAIN!

IIRC it's aboveground in the northeast of Stormbeach, in a chest in plain sight.
Grimrock 1 dungeon
