Ok, found the issue. Time passes really fast when resting and due to some precision errors monster's collision box could get left behind in certain rare circumstances. I bet you rested between the no-bug and bugged save games?

This issue will be fixed in the upcoming patch so that this problem should not happen anymore and also old saves will be automatically fixed.

Might be that I pressed r by mistake, did it somewhere while playing. And no trouble because I was anyways going to start over.

I'd like to suggest a small (and hopefully easy) improvement. Could it be possible to be able to map the "Hide all panels" hotkey please? Right now, it is mapped to ESC, but I don't find it very practical. When you are in the middle of a fight, those panels can really get annoying, and you want to hide them as quickly as possible. Regarding this, I think that using the space bar to hide all open panels would be very handy.
Do you think you could add this hotkey definition in the options, please? That way, everybody could choose the key they prefer.

Fight frogs in Keelbreach Bog at a ladder. Climb up the ladder, fight the frog until it's directly before you, then go down. The frog will now try to leap or attack but can't due to different height; you'll get a short blink of the frog at the bottom of the ladder trying this. Now move away from the ladder and try to move back =>> as long as the frog is at the top of the ladder you can't, invisible blocker!
Once the frog moves away the blocker is gone.
