Items lose their Special attack

So this has happened to me on two occasions. I have an item with a special attack, I use that special attack several times and then suddenly I discover that nothing happens when I'm holding down the right mouse button. I can still use the regular attack, but not the special attack. First time was with that wizard orb that has lightning bolt as it's special attack. The second time was with the Meteor Hammer. On both occasions this happened while I was in the bog and I think it happens while I'm fighting those giant frogs. I was able to detect it when it happened to my Meteor Hammer and reloaded from an autosave, which fixed the item so I was able to use the special attack. It did however happen again and now my meteor hammer is still broken at the latest autosave. Does anyone have a fix for this?

Out of charges?. Maybe you need to recharge those items. Just a wild guess.

I'm sorry, didn't even see that Meteor hammer had charges. Thanks a lot for the help though.
