I remember that Yahtzee offered this Idea on Extra Punctuation (from Escapist Magazine, for those who don't know it).

I think this would be kick ass! But I love twisted ideas.
You start all high and mighty... a Hero dropped in a dungeon. Then, your equipment degrades, you start to lose energy because you are only eating scraps, you start to go a little insane and lose sense of how to cast spells.
By the end, you can see the light at the end of the tunnel, having lost almost all hope, the last memory of your loved one fading... but the damn giant slugs are just too much for the rocks you can barely throw.
Hell yessssss! This idea of losing a sense of accomplishment wouldn't affect me... what better accomplishment than surviving!!
Reminds me of those games where you start off one level at the top of the world, getting knocked down, then trying to build up to that point again for the rest of the game.

Might be played around with by me or someone else in the future. What I personally would love to see would be a Scarab of Ra type mod. Hells horses that game was cool, but maybe it's because I was realllly young when I played it. Couldn't get it running on Mac emulators either..
