Permadeath would only be difficult if you limited the ability to save. I think if you were to watch my playthrough, counting only saved time I only had 2 deaths (fighter's and cube). A more challenging mode that limits the ability to save and has permanent death would be pretty cool, though.

I probly wouldnt do the perma death run b/c

I would know the boss battle would come down to luck with the way the enemies come after you.

A mod that:
refills all levels with monster-respawns after some time
+ a little clever placed 'bring-object-to-some-levels-above-to-'get-on'-further-down puzzles' from time to time
+ resurrection only possible at special altar-crystal in
SpoilerShow
secret chamber of starting room
+ only allows auto-save at crystals

Someone who knows the dungeon and monster layout completely from memory and has a lot of experience with the game on all other modes might want to attempt Permadeath. From how much the game forces the individual to explore and search, memorizing the entire layouts of the floors isn't that hard for someone who loves to play that way.
